Our Navigators include professional nurses (Nurse Navigators) and health and wellness coordinators (Navigators) who offer expert advice and provide knowledgeable direction to guide you at any stage of your life journey.  Because of the enormous amount of information, opinions and theories that are thrown at us, it is our goal to help filter out the noise and prioritize what is most needed for you, providing some peace of mind when it comes to your decision making.

It’s not easy to prepare for the unknown or the unanticipated, but with the help of a Healthspan Navigator, you can become more aware of possibilities and potentials, and take action to affect the outcomes you desire.

Premium

Only Healthspan Navigators Members are able to engage our services; we are not available to the general public.  We are committed to providing top tier service, reliably and dependably, with consistency for each of our HN Members.  Your dedicated Navigator will work with you, for as few or as many hours as you request, throughout your ongoing membership.

Partner

Much like financial planners or legal advisors, our Navigators are also trusted partners in your life journey, adjusting and coordinating plans as conditions and goals change.  And while we are not direct care service providers, we have vetted and established strategic partner relationships so that we can confidently recommend and introduce requested services when needed.

Coordination

It’s difficult to stay on top of all the appointments, tests and wellness checks that are needed to stay vital and in your best health possible.  We can help you make appointments, schedule tests and even accompany you to those meetings as you choose, facilitating communication and advocating for you both in the meeting and potentially afterward with your loved ones.  It is our goal to bring you some peace of mind in the midst of all the information that is out there by coordinating what is a priority to you.

Communication

One of the biggest and most common concerns when it comes to managing healthcare is the lack of accurate and shared communication between providers and different health entities. 

You find yourself having to bring everyone up to speed on your own when you potentially have to go to a doctor, or a specialist, or a lab, or a physical therapist, or a nutritionist, or a wellness clinic, or an urgent care, or a dentist, or a chiropractor, and so on and so on.  Your Navigator, with your permission and use of applicable platforms, will be able to update and share information across entities, ensuring important information is not missed.

It’s all about you!  Your life, health and wellness status, as well as your current goals, will be specific to you, and they will continue to change as you grow older.  We will walk that walk with you, helping you to make the choices that are best for you at every step along the way.

One stop shopping.  Remember that term?  Well, that’s us.  Think of your Navigator as the center of a wheel, who has the ability to not only address an array of individual life issues, but also plug in the resources that you may need to support those issues.  Your Navigator is not there to just complete a task, but to help assess all the ways that your health and wellness can be supported, improved and organized for the best overall results.
